A pattern we see often: a migraine patient comes in saying their usual tablet — paracetamol, ibuprofen, or even sumatriptan — does not work as well as it used to. They need more of it, or it takes longer to work, or the headache comes back the next day. This is not in their head. There is a medical explanation.

Medication-Overuse Headache (MOH)

When pain medication is taken on more than 10-15 days per month for headaches over a sustained period, the brain's pain pathways change. The very medication meant to relieve pain starts contributing to more frequent headaches. This condition is called medication-overuse headache (MOH), previously known as rebound headache. It affects people taking triptans, NSAIDs, paracetamol, and combination analgesics. It is one of the most underdiagnosed causes of daily or near-daily headache.

How to Suspect MOH

Signs that MOH may be developing: headaches have become more frequent (not just during migraines), you are taking pain medication more than 2-3 times a week, the medication works but headache returns within hours, and the headache is worst in the mornings. If this pattern sounds familiar, MOH should be discussed with a doctor before trying yet another pain medication.

Breaking the Cycle

Treating MOH requires a supervised withdrawal from the overused medication — which paradoxically causes a worsening of headaches for 1-3 weeks before improving. This cannot be done comfortably without medical support. Alongside withdrawal, preventive treatment is started: either medication (propranolol, amitriptyline, topiramate) or, in some patients, constitutional homeopathy as a preventive approach. The goal is fewer headache days per month, not complete elimination which is unrealistic.

If you are taking migraine pain medication more than twice a week, discuss this with a doctor urgently. Continuing down that path worsens the condition significantly over time. At Tulsi Homeo in Belgavi, we assess migraine frequency, medication use patterns, and help design a preventive approach rather than just treating each attack.

ಪದೇ ಪದೇ ಕೇಳುವ ಪ್ರಶ್ನೆಗಳು

How do I know if I have MOH or just frequent migraines?

The key sign is that headache frequency has increased over months, you are taking medication most days, and the headaches have changed character (more constant, less clearly episodic). Neurologist assessment is recommended if you are unsure.

Can I stop my pain medication abruptly?

Not advisable without medical guidance. Abrupt withdrawal causes a severe rebound headache for 1-2 weeks. A supervised, gradual withdrawal with medical support and bridging treatment is far more manageable.

Do preventive migraine medications really work?

Yes, evidence-based preventive medications (propranolol, amitriptyline, topiramate, sodium valproate) reduce migraine frequency by 50% or more in many patients. They take 8-12 weeks to show full effect. Neurologist consultation is recommended for severe or frequent migraines.
